This form is needed in order to confirm your reservation, along with a deposit.  By filling out this form, you are requesting Kids Sea Camp to make a reservation.  The deposit required for a confirmed reservation is $1,000 per room.  Please note the following form can be submitted electronically by filling it out and submitting  it  or you can also download and print the Application PDF. This along with your payment form, terms and Conditions form and Liability waivers can all be faxed or mailed in to Kids Sea Camp Inc.   We prefer check payments, however a credit card authorization PDF form is provided here, and on our PDF FORMS for all Destinations.

   Many family or individual medical insurance policies do not cover injuries related to certain specific activities. In particular, scuba accidents are often excluded in medical coverage.
